The controls

Work down the hierarchy — collective protection first, PPE last. PPE never carries the job on its own.

The RPD rule

Match workers to task risk. Verify licences before work. Use toolbox talks for changes, incidents or new controls.

No worker starts task-specific work until competency and induction requirements are met.

Licences and inductions get verified before the task — competency you can't prove is competency you don't have.

Stop & escalate

See any one of these? The job stops until it's fixed — no exceptions.

  • Competency can't be verified
  • Behaviour shows unsafe practice — remove worker from task
Stop work→ tell the Site Supervisor → don't restart until the control is back and the SWMS is reviewed.

What gets recorded

The job isn't done until the evidence exists. Live records go in Breadcrumb.
