Recurring findings and open actions are the early warning. Management review acts on them, not around them.

Stop & escalate
See any one of these? The job stops until it's fixed — no exceptions.
- Overdue high-risk actions
- Repeat provider failures
- Trends show controls aren't working
Stop work→ tell the Site Supervisor → don't restart until the control is back and the SWMS is reviewed.
