The controls

Work down the hierarchy — collective protection first, PPE last. PPE never carries the job on its own.

The RPD rule

Close simple actions in Breadcrumb. Move systemic findings, repeat issues and non-conformances to PIMS.

Management review must consider recurring findings, open actions and the effectiveness of controls.

Recurring findings and open actions are the early warning. Management review acts on them, not around them.

Stop & escalate

See any one of these? The job stops until it's fixed — no exceptions.

  • Overdue high-risk actions
  • Repeat provider failures
  • Trends show controls aren't working
Stop work→ tell the Site Supervisor → don't restart until the control is back and the SWMS is reviewed.
